‘Pre Authorization Payment’ [PAP] – Form

Pre-authorized payments are arrangements made with the students to automatically withdraw fees from their bank account on a specified date.

Please make sure to read below instructions before submitting the form.

For Status Cancellation and Stop Payment:

1. Student is required to notify by the last calendar day of the month to stop any future payments. 
2. Fee charged for the month without notice is not refundable.

Kindly also note:

1. Submit this form with a scanned void cheque or bank information file.
2. Any charges incurred by Al Huda Islamic Center of Canada Inc. due to insufficient funds at the time of withdrawal and/or other fees charged related to the automatic withdrawal of funds for the purpose of fee payment are the sole responsibility of the payer and will be passed on as such.
